Let’s get started! To make sure we are able to meet your needs and wants before starting our journey together, we need a few pieces of information.
Meet Nurse Practitioner Katie: free 20 minute meet and greet with Katie FNP-C to answer any of your questions before the initial consultation. Select Meet & Greet when booking.
Schedule appointment and submit intake forms before initial consultation
Initial consultation with Katie FNP-C in-person for O’ahu or Kaua’i residents or over telehealth for those off island or in Oregon
Continued care after the initial consultation we offer follow ups, acute care visits, as needed.
I offer functional medicine and conventional medicine to my patients, allowing the patient to make informed decisions for their own health. Functional medicine can be booked with insurance with more frequent visits to address all of your health needs, or cash pay for an hour long intake at $250. Pricing for follow up visits: Complex (45mins) $175, Routine (30mins) $150, and Simple (15mins) $75
