Getting started with Katie FNP-C

Let’s get started! To make sure we are able to meet your needs before starting our journey together, we need a few pieces of information. 

  1. Schedule appointment and submit intake forms before initial consultation either by the book now button or by calling the office at 808-658-8837

  2. Initial consultation with Katie FNP-C in-person for O’ahu or Kaua’i residents or over telehealth for those off island or in Oregon

  3. Continued care after the initial consultation we offer follow ups, acute care visits, as needed.
