Contact me if your carrier is not covered, I can provide a superbill that can be submitted to your insurance company for consideration of reimbursement. Cash pay is used for functional medicine add on appointments that are not reimbursable by insurance.

I require 24 hours notice to cancel appointments without penalty so that I can offer the appointment to other patients. Cancellations within 24 hours of appointment time will incur a $50 fee.

If you opt for natural options to support your health and/or supplements, these are not covered by health insurance companies and will be charged at the patient’s expense.

Routine lab work is billed and charged to your insurance company. Coverage is unique to each patient’s coverage plan and is ultimately the patient’s responsiblity. For functional medicine, some labs are out of pocket cost to the patient.
