Let’s get started! To make sure we are able to meet your needs and wants before starting our journey together, we need a few pieces of information.
Meet Nurse Practitioner Katie: free 20 minute meet and greet with Katie FNP-C to answer any of your questions before the initial consultation. Select Meet & Greet when booking.
Schedule appointment and submit intake forms before initial consultation
Initial consultation with Katie FNP-C in-person for O’ahu or Kaua’i residents or over Zoom for those off island or in Oregon
Continued care after the initial consultation we offer follow ups, acute care visits, as needed.
We work within a hybrid model offering insurance coverage for primary care and acute care visits as well and Functional Medicine visits for cash pay for those seeking more in depth evaluation and management of their care and optimization of their health.
